Once printed, please read the documents carefully, provide all necessary information, and sign where required. You should then return the signed documents to your broker. If you prefer to send them directly to us, please be sure to put the name of your broker on the cover page of the account form. You may then mail the completed forms to RCG's New Accounts Department at 216 W. Jackson Blvd., Chicago, IL 60606, or you can fax them to our New Accounts Department at (312) 795-7799.

If you would prefer to fill out our hard-copy account form booklets, please contact your broker for further information. Return completed booklets to your broker or RCG's New Accounts Department at 216 W. Jackson Blvd., Chicago, IL 60606. If you return the booklet directly to us, please be sure to include the name of your broker on the cover page of the booklet.
